Account Terms & Conditions: I understand that my facility can only have one WebPortal/LabOnline account for electronic test ordering and reporting. The person listed on this form agrees to be the facility point of contact.  As the facility point of contact, the person named is responsible for granting access to the WebPortal/LabOnline account to eligible staff within the facility. The facility also agrees to notify IBL if a change in point of contact occurs.
